Translation

One who meditates upon the Omniscient, Primeval One, Ruler and Creator of all, who is more subtle than an atom, whose nature is inconceivable, who is as refulgent as the sun, and who is beyond Tamas (primordial state of undifferentiated matter) — at the time of death, with a mind unwavering by the power of Yoga, being possessed of devotion, having focused the Vital Force (Prāṇa) between the eyebrows — reaches that same Divine Supreme Being.

Purport

Continuing from earlier shloka where Bhagvan Shri Krishna described that one should have a practice of meditating upon God. That habit will help human beings to remember God at the difficult moment of death. In this Shloka Bhagvan Shri Krishna gives more details.

Shri Krishna says that a human should first realize this supreme truth that I am omniscient, primeval source of all existence and creator of all. Human realizes this sooner is better. Such realization gives strength to the soul, though that strength he gains the power of yoga. By such yogic power of devotion, he is then not deviated away from Me, the God in any situation no matter if it is the moment of death.

The summary of the above two Shlokas and also Shloka#8 is that a human should try to realize the truth about God early in life and try to practice some form of devotion regularly. Such constant practice of devotion will make it 100% sure that the human ability to complete this journey of life smoothly and then achieve the supreme abode after this life.
